Notice of Multistate Vendor Security Incident Affecting SafeVoice Nevada; System Remains Operational
October 22, 2026
CARSON CITY, Nev. – The Nevada Department of Education (NDE) and the Nevada Department of Public Safety (DPS) are providing public notice of a data breach of SafeVoice Nevada caused by a multistate cybersecurity incident of technology vendor, Navigate360.
SafeVoice is Nevada’s anonymous reporting system that connects students and families with help when they have concerns about school safety or student well-being.
In response to the incident, NDE and DPS are taking action to protect SafeVoice and coordinating with state education leaders across the country to demand accountability and transparency from Navigate360.
Navigate360 has assured the state that SafeVoice remains fully operational and available 24/7 for students and families to report safety concerns.
“SafeVoice gives Nevada students a way to speak up when they are concerned about their own safety or the well-being of someone else, connecting those concerns with adults who can step in and help before harm occurs,” said Dr. Victor Wakefield, Superintendent of Public Instruction. “That trust is essential, and the compromise of sensitive information is unacceptable. We have pressed Navigate360 for answers, and we are working with state education leaders across the country to demand greater accountability and stronger protections for students. Nevada families deserve confidence that their information is secure, and we will continue taking action to protect that trust.”
“Protecting the safety of Nevadans is our highest priority, and that commitment extends to safeguarding sensitive information entrusted to programs like SafeVoice,” said George Togliatti, Director of the Nevada Department of Public Safety. “While the SafeVoice system remained operational throughout this period, any compromise of data is something we take extremely seriously. We are working closely with the Nevada Department of Education to keep the public informed, hold our vendors to the highest security standards, and maintain public confidence in this vital, life-saving tool.”
Navigate360 provided initial notification to NDE and DPS in spring 2026 that SafeVoice may have been affected by a criminal multistate cybersecurity incident occurring between November 2025 and March 2026, and that an independent forensic investigation was underway.
Although the incident drew Congressional inquiries, national media coverage, and a U.S. Department of Justice criminal investigation, Nevada-specific information remained limited.
This month, Navigate360 notified NDE and DPS that the forensic investigation concluded that approximately 10 percent of SafeVoice Nevada tips with personally identifiable information (PII) was compromised.
Nevada law (NRS 388.1454–388.1458) establishes strong protections for the anonymity of SafeVoice tips and the confidentiality of information submitted through the system.
Because the system is designed to allow individuals to report concerns without identifying themselves, NDE is unable to reliably identify and directly notify all affected individuals.
As a result, NDE is providing public notice in accordance with NRS 603A.220 through this release and the SafeVoice website. Navigate360 will separately issue public notice pursuant to a court order.
In response to this incident, NDE and DPS is working with school districts and local law enforcement to share information and provide guidance to support students and families.
The agencies will also post information on the SafeVoice website to ensure the public understands the potential impact and how to access assistance and available remedies through Navigate360.
Beyond Nevada, NDE is coordinating with state education leaders across the country whose students and schools have been affected by the same vendor incident. This effort reflects a shared commitment to protecting sensitive student information and ensuring vendors entrusted with that information meet the highest standards of security and responsiveness.
NDE and DPS will continue to press Navigate360 for answers and corrective action while working with other states to strengthen vendor accountability and safeguards for sensitive student information.
NDE, with support from the Governor’s Office, has also issued a Request for Information to evaluate alternative vendors and their security protections.
Navigate360 has created a call center through the firm HaystackID, who conducted the cyber forensics analysis, to address specific questions and concerns.
