Secondary Transition

Overview

Secondary transition is a results-oriented process that is focused on improving the academic and functional achievement of students with disabilities to assist the individual's movement from school to post-school living, learning, and working environments. The Nevada Department of Education Office of Inclusive Education's secondary transition efforts are grounded in evidence-based practices for post-school success and foster a systematic approach to increasing graduation rates and improving postsecondary outcomes through a greater focus on postsecondary preparation, recognizing the importance of partnership between students with disabilities, their families, the school, higher education and training programs, and outside agencies.

The Nevada Transitions Roadmap through Innovative Partnerships (Nevada TRIP) is empowering students with disabilities from elementary school through high school to enrolling in higher education, finding a job and future independence.

Nevada Vocational Rehabilitation

Nevada Vocational Rehabilitation (VR) can help Nevada students with disabilities transition to higher education and/or employment. Vocational Rehabilitation counselors are available to provide transition assistance to students as young as age 14 in middle school and those in high schools and alternative schools in Nevada. The transition program is a team effort involving students, parents/guardians, school staff, Vocational Rehabilitation counselors and transition coordinators.

Nevada Division on Career Development and Transition

The Nevada Division on Career Development and Transition (NV-DCDT) is the statewide organization in Nevada dedicated to assisting students with exceptionalities in achieving equality through career development and transition services. NV-DCDT works to help students with exceptionalities from early childhood through adulthood by implementing successful career awareness programs, career exploration, job preparation, and community living.

National Technical Assistance Center on Transition: The Collaborative

The National Technical Assistance Center on Transition: The Collaborative (NTACT:C) is a Technical Assistance Center co-funded by the U.S. Department of Education's Office of Special Education Programs (OSEP) and the Rehabilitation Services Administration (RSA). They provide information, tools, and support to assist multiple stakeholders in delivering effective services and instruction for secondary students and out of school youth with disabilities.
